HOHCS Service

Community Support Service

Community Support Service is a supportive service collectively provided by four service units of Haven of Hope. We aim to provide one-stop rehabilitation service and personal care to persons with disabilities and persons in need. We also strive to provide supportive services for carers of persons with disabilities to enhance their caring capacities.

Professional Rehabilitation Services

Provide a full range of therapies including physiotherapy, occupational therapy, alternative therapy, swallowing therapy, acupuncture moxibustion therapy and cognitive training.

District Support Centre

Provide Case Management services for the persons with disabilities and their families, individual and group training and support, community education, etc.

Scope of Service

  • Case management service including conducting need assessment, formulation of case plan and rendering counseling service for the persons with disabilities and their families so as to bridge them up with the community
  • Individual or group training programmes and support services for persons with disabilities to enhance their domestic living and community living skills
  • Social, recreational and personal development programmes for persons with disabilities to enrich their social life
  • Training programmes, educational courses, workshops, supportive services for family members / carers of persons with disabilities to enhance their caring capacities and relieve their stress
  • Drop-in service for persons with disabilities
  • Provision of community education service
  • Encourage community linkages and ongoing service promotion

Community Rehabilitation Day Centre

Haven of Hope offers one-stop integrated trans-disciplinary physical and psychosocial rehabilitation service. Our professional in our Community Rehabilitation Day Centre team includes physiotherapist, occupational therapist, nurse and social worker. Persons, aged 15 or above, who had undergone rehabilitation in hospital authority and still need further rehabilitation, are welcome to use our services. We strive to enhance service users’ physical functionand self-maintenance abilities, strengthen their domestic living and community living skills, help them restructure a healthy lifestyle and establish productive life roles so as to facilitate their integration into the community.

  • Time-limited center-based individual or group rehabilitation training programmes
  • Carers’ training programmes / educational courses / workshops
  • Community Integration programmes
  • Support services: organizing mutual support groups for service users, the launch of public education programmes and the provision of consultation services to support groups and rehabilitation units in the community

Day Care Service for Persons with Severe Disabilies

Day Care Service for Persons with Severe Disabilities provides centre-based day care for persons with severe physical or intellectual disabilities. We aim to provide regular day care including nursing, rehabilitation, social and personal care services so as to enhance opportunities of persons with severe disabilities to continue living in the community.

Scope of Service

Provide service for 1) Persons with severe intellectual or physical disabilities aged 15 to 59 and *2) Aged persons with intellectual disabilities aged 15 to 59:

  • Day care including meal;
  • Nursing care and personal care
  • Maintenance programs on physical function and basic living skills
  • Social and recreational activities

In compliance with the Personal Data (Privacy) Ordinance, Haven of Hope Christian Service (HOHCS) and/ or its service units is committed to safeguard the personal information of web users. Personal data, including but not limited to name, telephone number, facsimile number, address, email address, record of donation will only be collected to organize, coordinate and carry out activities to support or encourage charitable and /or health related works organized and undertaken by HOHCS and/ or its service units and to publicize and promote such works. All reasonably practicable actions shall be taken to ensure that personal data collected and retained are accurate and securely protected. When web users visit the website of HOHCS, our service provider will only record their IP address and the pages browsed. No personally identifiable information shall be collected. All personal information that web user provides to HOHCS is secured on HOHCS's server with restricted access. HOHCS reserves the right to use agents, who are bound by strict confidentiality guideline, to perform storage and processing functions. Online Registration / Online Donation

When web users register or donate, their personal information will be securely encrypted, in which credit card information will only be collected by the Visa/Master companies via their payment gateway for the purpose of processing the donation payment. HOHCS will not disclose any of such information. Other personal information such as donor's name, telephone number, facsimile number, address, email address and record of donation will only be used for the purposes of issuing and sending of donation receipts and mailing of promotional materials by HOHCS.

 

Update and Removal
